About High Blood Pressure

Do you have high blood pressure? About 1 in every 3 American adults does, but the signs and symptoms aren’t always easy to recognize until serious problems arise. That’s why screening is so important. Also called “hypertension,” high blood pressure is a medical condition that can put you at risk for heart disease, stroke, and other serious illnesses.

But there is good news. We can help you reduce your risk of high blood pressure. If you already have hypertension, we can help you lower your blood pressure. You’ll find more information on this site about what causes high blood pressure, how to prevent it, how to correct it, and where to go for help.

Eligibility Requirements

Anyone may contact us for information about high blood pressure and for screening. However, once someone is diagnosed with high blood pressure, there are certain eligibility requirements for treatment in our Blood Pressure Clinic. The requirements are:

• Adult must have high blood pressure and low to moderate income (exact income guidelines available at the clinic)
• Must be a resident of Chatham or Effingham County
• Must bring in proof of household income such as: pay stubs, Social Security check, retirement, pension, child support, disability benefits, etc.

Please place all your current prescription medications (including over-the-counter medications) in a bag and bring them to your appointment.
